Job Opening: Oncology Nurse Navigator
Location: Mississauga, ON, CANADA

Job Summary:


The Nurse Navigator, under the direction of the Programs Manager, is responsible for the development of a learning based patient assistance program geared towards Oncology patients in the GTA and London. The successful candidate will ensure compliance with internal and external standards of practice. She or he will help develop the program and then coordinate clinical education and in-service activities as requested.

Job Duties and Responsibilities:


. Assess and then Arrange for the initial and ongoing Patient education sessions for the COPE patient assistance program, for Oncology patients at 5 identified key Cancer Care Centers
. Develop Education Plans for Oncology patients, based on the individual needs of each identified Cancer Care Center
. Create/Maintain relationships with key stake holders in the identified Cancer Care Centers. Including the in-house Nurse Navigator, Medical Oncologists, Oncology Nurses and Pharmacists
. Lead or Assist with the integration of client satisfaction initiatives; such as surveys to obtain qualitative information on how the program is running
. Participate in business development as appropriate; assist with the development of contractual agreements for client care and service as requested
. Promote Bayshore Home Health through participation in local healthcare or business committees as well as community events
. Participate in ongoing internal and/or external continuing education activities
. Adhere to Bayshore Policies and Procedures
. Maintains Confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel
. Complete other tasks as requested

Job Experience:

At least five years of recent experience with progressive responsibilities, as an Oncology Nurse or Nurse Navigator, preferably in a hospital setting. Strong track record in leading the development and evaluation of patient programs; demonstrated ability to handle all aspects of human resources and interpret company policies and procedures. Experience in business development and CHE/CME development preferred.

Education:
Graduation from an accredited Licensed/Registered Nursing Program; current registration with the provincial regulating body, current Oncology Nursing accreditation.
